Unfortunately, despite our best efforts to accommodate your changing schedules, we have encountered a significant number of cancellations for \_ sessions, \_ has arrived for \_ out of the past \_ sessions. Due to this high rate of cancellations, we are unable to continue facilitating therapy sessions for \_.

We understand that unforeseen circumstances may arise, but consistent attendance is essential for effectiveness and progression aligning with \_ NDIS goals. We are committed to ensuring that each client has fair access to our services,

Thank you for your consideration and planned attendance with your scheduled Hiya Health Practitioner thus far.

Repeated Cancellations / Our Attendance Expectations

Hiya Health reserves the right to offer a client's session time to another client if a frequent pattern of 3 or more cancellations occur. Hiya Health has a Cancellation and Attendance Policy that assists us to best manage each of our Practitioner's diaries ongoing to ensure we're enabling access to clients who need us, and are able to commit to attending planned scheduled appointments with us.

We've noticed it has been difficult for \_ to attend their scheduled appointments with the frequency required, and thus we are needing to cancel any ongoing scheduled appointments for \_ that are currently committed to within the Practitioner's diaries.

Infrequent or sporadic attendance to scheduled appointments not only impacts \_'s ability to progress and reach their desired goals, but also causes significant financial strain on our business and to the practitioner individually.

If in the future your circumstances change, and \_ is able to commit to resuming services with us in the planned frequency mutually agreed, we'd love to see you back in clinic again.
