Please find the \_ Service Agreement attached to this email for client, \_ \_. We understand that this client is managed by the Office of the Public Guardian, and we will book subsequent appointments upon receiving the endorsement confirmation.

This Service Agreement has been drawn up based on the assessing Practitioner's recommended Schedule of Support. Please note that our Practitioners have no insight relating to the extended NDIS Plan Budget and Spend for the Client.

It is therefore imperative we are advised as soon as possible if the recommended Schedule of Support is exceeding the planned allocated budget for the Client.

Please contact Hiya Health at reception@hiya.health for any Service Agreement amendments, and changes to the frequency of scheduled services that have been planned and allocated for this Client.
