A few DVA clients qualify for DVA to provide Taxi’s for them to attend their appointments at Hiya Health. Taxi’s are booked in the final week of one month for the entirety of the following month. The email is sent to
Good (Morning/Afternoon),
Please find below Hiya Health'sTaxirequest for (Client_Name) to attend their DVA Allied Health appointments at (Clinic_Location):
(Client Name) –(DVA Number) Phone Number:(Client’s phone number) Pick Up:(Client home address) To:(Clinic Name) & (Clinic address)
Please note: the times specified below are the PICKUP times (not the appointment times) - the DVA client will be ready and waiting for their taxi at these times - many thanks:
(Client_Name) will require a taxi to pick them up EVERY (Day) from (Time_to_Time inclusive, at:- PICKUP: pick up from Home Address at (Time)
- RETURN: pickup from Hiya Health (Clinic_Location) at (Time)
The appointment is scheduled for (Time) (Weekly/Fortnightly) on (Day), however, due to issues with the Taxi’s getting (Client_Name) to their appointment on time, we specify the pick up times and request that these be the times the taxi is ordered for - thank you.

DVA also have a 1800 550 455 number to cancel taxis if client is unwell.
The clients who qualify for DVA taxis are required to be 80+ years old.
Another option if DVA clients ask is if they pay for their own taxi and submit their receipts to DVA
